J-1 Students and Scholars under Columbia's J-1 sponsorship: SEVIS Report Form.
- If your DS-2019 was not issued by Columbia, please do not submit this form. If you received your DS-2019 from another J-1 Exchange Visitor program sponsor, such as IIE for Fulbright, follow their guidance for reporting your arrival to them.
If you're married: The F-2 or J-2 SEVIS records of your dependent(s) are automatically activated with yours. Your visa dependents do not need to complete a registration form but you should check to see that their I-94 arrival/departure record to the U.S. is accurate. They should be admitted with the notation of "J-2 until D/S". D/S refers to "duration of status".
